What we found on Optimal Health Medical Group

We reviewed FDA enforcement records, DEA registration, HIPAA breach records, and NPI registration.

Optimal Health Medical Group has no FDA, OIG, or DEA actions in the records we checked. It lists named physicians and appears to serve 1 state. We did not find public pricing, and the pharmacy relationship should be confirmed directly.

What this means

Optimal Health Medical Group has more public information available than many providers in this category we track. The main gap is pricing: we could not confirm the all-in monthly cost from public information. Before signing up, ask about medication cost, consult fees, shipping, cancellation terms, and which pharmacy fills the prescription.

Why this score: 4 of 6 checks complete. Each links to the public source it was verified against.

A check we haven’t completed yet is not a mark against this provider. It just means we’re still sourcing that record. The score reflects what we’ve confirmed so far, and updates as more checks come in.

Our take

Vial Editorial

Based on the records and public materials we reviewed.

Optimal Health Medical Group is a La Jolla, California practice offering testosterone and hormone therapy, peptides and longevity care, largely by telehealth, through monthly membership tiers. Its notable feature is a single physician at its center: the site names its founder, Michael Nielsen, MD, and describes his training at UC San Diego. The drawbacks are thin independent evidence, a site with visible gaps, and a state medical board record for the physician, set out below.

4 of 6 key checks found
2 important details still need to be confirmed directly.

What it costs

Care is sold as a monthly membership, from $250 to $3,000 a month. Core is $250 a month for email and phone access with a 48-hour response time, one lab a year that the patient pays for, a personalized treatment plan, a la carte access to treatments and an annual video consultation. Concierge is $1,000 a month for email, phone and video access with a 24-hour response time, two labs a year paid by insurance or the patient, discounted access to hormones and peptides, and monthly telehealth consultations. Longevity is $3,000 a month for email, video and in-person access with a 12-hour response time, quarterly lab work with insurance included, monthly in-person or video consultations, and advanced diagnostics including MRI and CT scans. The price of the medication itself is not published on the pages we checked. The practice says no insurance is required because it operates outside the insurance system, and that you are never locked in and can pause or cancel by contacting the team. Two statements on the same page do not fit together: the FAQ says the entire process is powered by telehealth and you never have to visit a physical clinic, while the Longevity tier promises in-person consultations. The site's disclaimer page describes credit tradelines rather than medical care, so it offers no medical disclaimer at all.

What patients report

There is almost no independent review evidence. Dr. Michael Nielsen's Healthgrades profile, which is unclaimed, shows 4.0 stars from four ratings, 75 percent five-star and 25 percent one-star, with a single written review praising him as thorough in reviewing labs and responsive. Four ratings cannot describe a practice. Reddit was not sampled.

What Vial measures, and what we don’t

We evaluate public records: federal enforcement history, registrations, certifications, and how transparent a provider is about its clinicians, pharmacies, and pricing. We don’t score bedside manner, clinical outcomes, or customer satisfaction, and we don’t collect user reviews.
