About Teduglutide

Teduglutide is a GLP-2 analog FDA-approved for short bowel syndrome (SBS) in adults and pediatric patients (brand: Gattex, NDA 203441). It promotes intestinal adaptation by stimulating crypt cell proliferation and reducing intestinal transit time, enabling some SBS patients to reduce or eliminate parenteral nutrition dependence. Note: teduglutide is the approved synthetic analog of the native GLP-2 peptide; GLP-2 (the unmodified native hormone) is a separate entry in this directory with RUO status.

Science

Mechanism of Action

Binds GLP-2 receptors on enteroendocrine L-cells and subepithelial myofibroblasts, triggering EGF, KGF, and IGF-1 release. This paracrine cascade promotes intestinal crypt cell proliferation, villus growth, and tight-junction integrity, increasing absorptive surface area. The alaGlu substitution at position 2 prevents DPP-4 cleavage, extending half-life from ~7 minutes (native GLP-2) to ~1.3 hours.

Dosing

Typical Protocol

0.05 mg/kg/day subcutaneous injection once daily. Dose individualized by weight; titration over weeks is standard. Colonoscopy required at baseline (polyp risk). Response assessment at 6 months; continued use requires demonstrable clinical benefit. Administered under physician supervision.
