About Teriparatide
Teriparatide is a recombinant fragment of parathyroid hormone (PTH 1–34) FDA-approved for osteoporosis in postmenopausal women, men, and glucocorticoid-induced osteoporosis (brand: Forteo, NDA 021318). It is the only approved anabolic bone-forming agent that works by stimulating osteoblast activity rather than suppressing osteoclasts. It is used off-label by longevity physicians for accelerated bone-density recovery. Administered by daily subcutaneous injection; duration is capped at 24 months (lifetime) due to osteosarcoma signal in high-dose animal studies.

Mechanism of Action
Binds PTH/PTHrP receptor (PTH1R) on osteoblasts, activating adenylyl cyclase and protein kinase A pathways. Pulsatile (once-daily) administration preferentially drives osteoblast proliferation and differentiation over apoptosis, yielding net bone formation. Downstream: increases serum calcium and 1,25-OH vitamin D; stimulates renal phosphate excretion. The anabolic window is lost with continuous exposure — daily injection is mechanistically required.
Dosing
Typical Protocol
20 mcg subcutaneously once daily, injected into thigh or abdomen. Maximum cumulative lifetime exposure: 24 months (FDA label limit due to osteosarcoma risk in rodent studies at high doses; clinical relevance in humans is uncertain but label restriction stands). Typically followed by antiresorptive therapy (bisphosphonate or denosumab) to consolidate bone gains. Requires physician supervision and baseline bone-density scan.
