Fifty 410 is a Texas telehealth service selling compounded tirzepatide and semaglutide. Tirzepatide is sold only as a prepaid three-month supply: $399, $499 or $549 depending on the dose tier. They advertise it from $133 a month; the smallest charge is $399. Semaglutide is $199 month to month. Nothing renews automatically. Refunds stop once the pharmacy starts compounding your order.
The FDA issued a warning letter to Aspen Aesthetics dba Fifty 410 on February 20, 2026. We found no close-out letter for it in FDA records checked on September 23, 2026. The letter is published at https://www.fda.gov/inspections-compliance-enforcement-and-criminal-investigations/warning-letters/aspen-aesthetics-dba-fifty-410-721481-02202026. We couldn't find which pharmacy they use or who prescribes. Trustpilot reviewers give them 4.8 from 6,119 reviews. The Better Business Bureau gives them an F, with 21 complaints in three years, 8 unanswered.
What it costs
There is no membership or subscription. Fifty 410 says the price you see includes medication, provider consultation and shipping, with no hidden fees, and no card is taken up front: a payment link is sent only if a licensed provider issues a prescription, and the medication for the whole program then ships at once. Compounded tirzepatide is sold as 3-month programs: the Starter Program at $399 in total, or $133 a month, the Mid Dose Program at $499, or $166 a month, and the High Dose Program at $549, or $183 a month. Compounded semaglutide programs start at $199 a month. The homepage lists GLP-1 programs as starting at $199 a month, which is the semaglutide price, though the tirzepatide program starts lower. Fifty 410 does not bill insurance directly. FSA and HSA funds may be eligible, and Afterpay and Affirm are offered. The refund policy, last updated September 8, 2026, turns on the pharmacy: a full refund is available after payment until compounding begins, there is no routine refund once compounding has begun, and the dispensing pharmacy's order status decides when that is. A cancellation counts only once Fifty 410 confirms it was received before that point.

The lower-rated reviews describe communication gaps, including having to ask for delivery estimates and tracking information after paying. The Better Business Bureau file reads very differently: an F grade, which the BBB attributes to concerns with the business's practices and advertising issues it found, no accreditation, 21 complaints closed in three years, seven customer reviews, and the file opened July 19, 2024.
