TRT clinics can sell the same injections while operating very different medical practices. Vial reviews 319 providers on the parts that do not show up in the ad: how the diagnosis gets made, what gets monitored afterward, who the physician is, and where the testosterone comes from.

The treatment

TRT replaces testosterone when the body does not produce enough

Testosterone replacement therapy raises testosterone levels using injections, gels, patches, pellets or other prescribed formulations.

FDA-approved testosterone products treat men with low testosterone tied to an established medical condition. The FDA has not approved them simply to reverse normal age-related decline.

A proper evaluation looks beyond a single low result.

Clinicians should consider:

  • Symptoms consistent with testosterone deficiency.
  • More than one morning testosterone measurement.
  • Possible causes of low testosterone.
  • Fertility plans.

The same prescription can come with very different care

The medication is only one piece.

A high-touch clinic may review medical history, repeat laboratory testing, discuss fertility and adjust treatment over time. A volume-driven platform may use a standard protocol with limited clinician contact.

Both may call the service "physician supervised."

That phrase tells you almost nothing by itself.

The regulation

Testosterone is a Schedule III controlled substance

Federal law classifies testosterone as a Schedule III controlled substance because it has accepted medical uses and a recognized potential for misuse and dependence.

That status creates real prescribing and dispensing obligations.

The clinician must hold the licenses and controlled-substance authority required to prescribe where the patient receives care. The pharmacy must also hold the proper licenses to dispense and ship into that state.

A professional website does not prove either one.

TRT carries risks that require monitoring

Testosterone can increase red blood cell production. In some patients, hematocrit rises enough to increase clinical concern and force a dose change or treatment pause.

Current product labeling also addresses risks involving:

  • Blood-pressure increases.
  • Reduced sperm production.
  • Venous thromboembolism.
  • Worsening sleep apnea.
  • Prostate-related symptoms.
  • Edema.
  • Lipid changes.
  • Abuse and dependence.

TRT should not operate like a supplement subscription. The follow-up plan matters as much as the first prescription.

A named pharmacy

Patients should know who dispenses the testosterone.

Verify:

  • State pharmacy licenses.
  • Authority to ship into your state.
  • Accreditation where relevant.
  • Recall and enforcement history.
  • Whether the medication is FDA-approved or compounded.
  • Any ownership relationship between the clinic and pharmacy.

A clinic that refuses to name its pharmacy blocks you from checking the most important part of its supply chain.

A real monitoring plan

TRT requires more than testing total testosterone once or twice a year.

Ask which markers the provider follows and how often. Depending on the patient, that may include:

  • Total and free testosterone.
  • Hematocrit and hemoglobin.
  • Prostate-specific antigen.
  • Estradiol when clinically relevant.
  • Lipids and blood pressure.
  • Symptoms, side effects and fertility goals.

More testing does not automatically mean better medicine. The clinic should be able to explain why each test changes treatment.

The full monthly cost

TRT pricing often separates the membership from the medication.

Laboratory testing, shipping, supplies, follow-up visits and additional drugs may cost extra. Some clinics also add medications such as anastrozole or hCG to standard packages before establishing whether the patient needs them.

Ask for the expected total before enrolling.

Testosterone enanthate is a long-acting esterified form of testosterone with a half-life of 4–5 days, making weekly or twice-weekly injections standard. It is FDA-approved for hypogonadism and is one of the most prescribed TRT formulations in the US. In optimization medicine it is used to restore and maintain testosterone to youthful physiologic levels. It requires injection but offers stable serum levels with proper dosing frequency.

Human chorionic gonadotropin (HCG) is an LH-mimicking hormone used in TRT to maintain testicular function, prevent atrophy, and preserve fertility during exogenous testosterone therapy. It is also used in male hypogonadism as a monotherapy to stimulate endogenous testosterone production. Compounded HCG remains available after FDA discontinued most branded injectable formulations.
